Please use this form for a community/feral cat admission to LifeLine's Spay Neuter center at 2533 Sullivan Rd. in College Park. 

Fill it out in advance to save time.  Upon arrival, take the form in the receptionist.  Leave kitties in your car until called in.  They may begin taking community cats at 8:30 AM.  Pickup is 3:30 PM.  Late charge applies after 4:30.

If you have a confirmed appointment booked through Tri Cities Friends of Kitties at LifeLine/College Park, get the cat(s) there at 7:30 AM. If you haven't filled out the form, go in the building and get the form and fill it out and let them know you are with Tri Cities.  Pickup in afternoon is 3:30.
