Please use this form for a community/feral cat admission to LifeLine's Spay Neuter center at 2533 Sullivan Rd. in College Park. 

Fill it out in advance to save time and possibly get a better shot at the first-come/first-serve intake order.

Upon arrival, take the form in the receptionist.  Leave kitties in your car until called in.  Intake is generally at 8:30 AM.  Pickup is 4:30 PM.  Late charge applies after 4:30.
